RMA Number Request Form

Important instructions
Use this form if you have bought hardware from ProVu Communications which has gone faulty.
Please provide as much information as possible about the fault.
A member of our technical support team will then contact you (usually via email) as soon as possible with further instruction.
Where possible, ProVu will always endeavour to repair RMA'd units. Where items cannot be repaired, a replacement or a credit can be issued, in line with ProVu's return policy. Please indicate on the form below your preference, should your RMA be found to be beyond repair.
Any requested credits will only be authorised in accordance with our Returns Policy.
Where an RMA is returned to ProVu and no faults can be replicated, the cost of the return carriage will be invoiced to the customer. An inspection fee may also be issued at ProVu's discretion.

If you have items such as power supplies or headsets which do not have serial numbers to submit, please type the word NOSERIAL into the serial number box for each item which doesn’t have a serial number.
Items which are out of warranty may be returned for repair for an agreed fee- quotes can be provided upon request. As this service is chargeable, ProVu may request an official purchase order to be raised.
